Job Title: Coordinator – Community Health Worker
Job Description
NyECC is seeking a Community Health Worker(s) to work directly with community members in need and to work with community partners to build systems and relationships to address community issues related to community wellness, food security, substance misuse, and behavioral health. They will assess current living situations of individuals and/or within the community, make referrals / create referral systems, and provide linkages to services for community members; along with providing information and education to the community, gathering information to understand systems and community needs and resources.
Community Health Workers will take on a variety of tasks which may include some or all of the following: providing support and education regarding nutrition, diabetes, crib safety, car seat safety, family planning, opioid awareness, substance abuse, mental health awareness, and suicide prevention.

Key Skills / Experience
- Understanding of Case Management and/or community assessment
- Strong ability to relate to teenagers, young adults, seniors, and other community members in crisis
- Experience with creating linkages to services, and helping people in a respectful manner.
Hours Per Week
Part-Time and Full-Time Positions
Pay Range
$14.50 /hour (non-exempt)
Benefits: 99% of Medical, Retirement after one Year, major holidays, sick and vacation time
Minimum Education Requirement
- High School Diploma required; Social Service, Criminal Justice, Social Work, Public Health or other similar degree preferred.
- Computer Competent
- NV Driver’s License / Reliable Transportation
